Electronic Delivery of Form 1095-C
We are providing this notice to alert you of an important change regarding the distribution of Form 1095-C. Bosque County Hospital District is no longer required to automatically mail the Form 1095-C to individuals.
The forms are available by contacting Human Resources. A form will be printed and either emailed, mailed, or you can pick it up at the hospital.
